plan designs include PPO options, POS options and traditional indemnity plan options.
These different plan designs allow the financial institution to choose the plan of

the budget allocated by the bank. The plan design choices include network only plans, as
well as in and out-of-network plans with varying co-pays and deductibles. Also included in
